Athlete Physicals @ School / $10 cash; the 'Physicals Form' needs to be turned into Coach Mihecoby by May 17. The additional athletic forms online from GISD Athletics need to be submitted before summer workouts begin.

There are a variety of forms that must be submitted online before a student can participate in school athletics.
- Submit all forms online through the Athletic forms system. (not set up yet as of 4/1/24 for the 24-25 school year)
- Review the Athletics forms system instructions page to help minimize any delays of your student's participation.
